abstract

objective bacterial meningitis is still a life threatening epidemiological problem especially in many developing countries; considering its dire consequences, its prompt and accurate diagnosis has become a priority for clinicians. because of the various limitations of conventionally used laboratory techniques, we evaluated and compared the diagnostic utility of c-reactive protein(crp) and lactate dehydrogenase (ldh)in serum and cerebrospinal fluid (csf)in the diagnosis of bacterial meningitis and its effectivity in distinguishing it from aseptic meningitis (ap). material and methods a total of 125 pediatric cases, aged between 1 month and 12 years, including patients with bacterial meningitis (n=45), aseptic meningitis (n=42) and a control group (n=38), were retrospectively analyzed on the basis of data from the initial clinical examinations. cultures, smears and other common serum and csf indices were compared with serum and csf crp levels and ldh activity. results compared with each of the other variables, there were significant differences in the mean values of serum-crp, csf-glucose, csf-ldh and csf/serum ldh ratio between the bacterial and aseptic meningitis groups (p conclusion in the presence of a normal crp titration and low glucose level in csf, bacterial meningitis is excluded, whereas elevated level of csf-ldh activity is a valid confirmatory predictor of bm. in addition, combination of these three tests with serum crp is far more effective than the separate determination of any of these parameters.

A STUDY COMPARING CEREBROSPINAL FLUID LACTATE LEVELS IN CHILDHOOD CULTURE POSITIVE AND CULTURE NEGATIVE MENINGITIS

Despite recent innovations in the laboratory diagnosis of bacterial from nonbacterial meningitis, solid data-necessary for early determination of bacterial meningitis (BM) before organism growth in the culture medium-are missing. Therefore cerebrospinal fluid (CSF) lactate levels were evaluated as a possible means of differentiating the two clinical entities. This was a retrospective study....

Is there any Correlation between Cerebrospinal Fluid and Serum C-reactive Protein in Neonates Suspected to Meningitis?

Background: Meningitis is a common life threatening infection in neonatal period. Diagnostic value of CSF-CRP level in bacterial meningitis in children and adults has been studied worldwide, but there are limited studies on CSF-CRP in neonatal meningitis. This study conducted to assess any relation between CSF-CRP and plasma CRP levels and abnormal CSF findings in neonates suspected to meningit...

Estimation of Lactate Dehydrogenase Activity in cerebrospinal fluid in different types of meningitis

Objective: Meningitis is life-threatening condition and examination of the Cerebrospinal fluid (CSF) may not give a precise diagnosis and prognosis of different types of meningitis. Bacterial meningitis is still a very common problem especially in many developing countries. The aims of study are to estimate the importance and significance of Lactate Dehydrogenase (LDH), protein...

Comparison of cerebrospinal fluid C-reactive protein and lactate for diagnosis of meningitis.

Levels of C-reactive protein and lactate were determined on 562 consecutive cerebrospinal fluid (CSF) samples from adult patients with a wide variety of central nervous system diseases to compare the sensitivity and specificity of CSF lactate and C-reactive protein for the rapid diagnosis of bacterial meningitis. Neither test alone, together, or in combination with elevated CSF leukocyte count ...

Increased cerebrospinal fluid lactate and early diagnosis of bacterial meningitis.

CLINICAL CHEMISTRY, Vol. 25, No. 5, 1979 809 has yielded correlations with active malignant disease (1, 3). However, the lack of specificity of changes in concentration of these components has decreased their clinical diagnostic usefulness. By using cellulose acetate membranes with Mylar backing (4), we have been able to separate the a2-globulin region into several subregions. In Figure 1, we s...
